Successful treatments for allergic bronchopulmonary aspergillosis in non-cystic fibrosis children: a report of two cases and a literature review

چکیده
Allergic bronchopulmonary aspergillosis (ABPA) is a disease induced by an exaggerated immune response to Aspergillus fumigatus, often occurring in susceptible adult patients with asthma and cystic fibrosis (CF). Proper treatments can alleviate symptoms, reduce pulmonary infiltrates and prevent progression of lung destruction. However, treatment experiences in ABPA children without CF are limited. Here we present two cases of successful treatments of ABPA in non-CF pediatric patients in China. We also present a literature review of ABPA treatments in 15 children without CF from 1971 to 2015. Based on these two cases, and our literature review, we recommend long-term low-dose oral corticosteroid therapy combined with antifungal agents along with close monitoring for these children, and suggest that anti-IgE therapy is not necessary.

Two cases of non-cystic fibrosis (CF) bronchiectasis with allergic bronchopulmonary aspergillosis
Allergic bronchopulmonary aspergillosis (ABPA) is a complex hypersensitivity reaction in patients with asthma or cystic fibrosis (CF), which is associated with bronchi colonized by the fungus Aspergillus species, most often Aspergillus fumigatus. ABPA is an important consideration for asthmatics that do not respond to asthma management or with recurrent chest infections and deteriorating lung f...

BACKGROUND Allergic bronchopulmonary aspergillosis is a serious complication of cystic fibrosis and may be difficult to diagnose. The aim of this study was to define the usefulness of measuring total IgE compared with other major criteria in the diagnosis of allergic bronchopulmonary aspergillosis in children with cystic fibrosis. METHODS A retrospective analysis was carried out of the case r...

OBJECTIVES Allergic bronchopulmonary aspergillosis (ABPA) is a common complication in patients with cystic fibrosis. This cross-sectional study was planned to determine the prevalence and risk factors for ABPA in Indian children with cystic fibrosis. Asthma with allergic bronchopulmonary aspergillosis (ABPA), a hypersensitivity disease of the lungs due to an immune response to Aspergillus fumigattus (Af) antigens, is rarely seen in children, other than complicating cystic fibrosis. We present the case of a 14 - year- old female teenager with cystic fibrosis (CF), admitted in our hospital with respiratory failure and persistent cyanosis. Che...

Following lung transplantation for end-stage cystic fibrosis, two male patients presented with shortness of breath, peripheral blood eosinophilia and segmental lung collapse. At bronchoscopy, each had bronchial mucous plugging containing Aspergillus fumigatus. This finding was associated with a systemic eosinophilia and skin test positivity to Aspergillus. Augmented steroid therapy resulted in ...
